WORKERS’ COMPENSATION INFORMATION

The goal of the Pleasant Valley Joint Union Elementary School School District is to ensure that appropriate benefits are paid promptly and accurately to employees with work-related injuries/illnesses. Workers’ Compensation is a state-mandated program, funded completely by the employer, and is designed to guarantee the payment of specified benefits to employees who are injured on the job. The District is self-insured for workers’ compensation benefits, and such benefits are paid using district funds.

ALL EMPLOYEES MUST REPORT A WORK-RELATED INJURY WITHIN 24 HOURS OF THE INJURY OCCURRING. If available, notify the Superintendent and/or Lynn Ferrin (805) 467-3453 for authorization for treatment. After-hour injuries ONLY may seek treatment at Med Post, 500 1st St., Paso Robles, CA 93446. If closed, seek treatment at the nearest emergency room, then report to Human Resources the following business day.

Before going to the doctor, however, call SIPE Triage Nurse Program at 1-855-519-8472 to triage injury and provide appropriate direction, and if needed, initiate Worker's Comp paperwork with MedPost, CareWorks, will take care of it for you. 

IMMEDIATELY after your appointment at MedPost turn in your paperwork to Lynn Ferrin.

The Superintendent or designee and the injured employee will complete the Employee Accident Report. A copy of the SIPE Accident and Investigation Report must be done within 24 hours. Do not delay submitting forms due to lack of signature. 

Employees not seeking medical assistance must still call SIPE Triage Nurse Program and include a Refusal of Medical Treatment Form with the Employee Accident Form. Should the employee later want to seek medical assistance, they can follow up with CareWorks 24/7 for easier, faster service than trying to hunt down the Superintendent and get their referral to the district's medical provider at that moment. 

EMPLOYEE LEAVES OF ABSENCE

The Pleasant Valley Joint Union Elementary School District offers a variety of leaves options to eligible employees such as Catastrophic Leave, Extended Illness Leave, Family Medical Leave (FMLA), Industrial Accident Leave, Unpaid Leave of Absence (LOA), etc.

Employees are required to contact the Superintendent if an employee may need to be absent from work for an extended period of time (5+ days) due to injury/illness of the employee or the employee’s immediate family.  It is requested that any foreseeable leaves be requested 30 days before the beginning of the leave.  

All requests for a leave of absence must be submitted to the Superintendent and/or Lynn Ferrin. The application to request a leave of absence is available by clicking the link HERE

NOTE: Any employee requesting a leave of absence* (5+ days) must complete the leaves request form within 30 days of the leave commencing. If your leave is due to medical reasons you will need to have your physician complete the Physician’s Healthcare Certification form and turn that in with your completed paperwork. Health benefits may be impacted when an employee has no paid leave available.
